What You'll Do
As an RN Nurse Supervisor, you will:
+ Oversee healthcare services, clinical interventions, and ongoing medical needs for program members
+ Participate in Individual Support Plans (ISPs) and interdisciplinary case conferences
+ Coordinate medical care with primary care providers and external partners
+ Advocate for the health, safety, and well-being of each program member
+ Conduct pre-admission reviews, assessments, and develop AFC plans of care
+ Ensure regulatory compliance and maintain accurate, confidential medical records
+ Recruit, train, supervise, and support AFC caregivers
+ Perform quality assurance activities, including audits and policy reviews
+ Collaborate with multiple agencies to ensure coordinated, high-quality care
+ Maintain all required certifications and follow safety protocols
What You Bring
Required Qualifications:
+ Active RN license in the state of Massachusetts
+ Minimum of 2 years of recent, hands-on experience with elderly or disabled adults
+ Experience working within AFC or DDS programs
+ Strong communication, organization, and documentation skills
+ Ability to educate and train adult learners
+ Basic computer proficiency and comfort using Zoom and EMR systems
+ Valid driver's license, reliable personal vehicle, current insurance, and annual vehicle inspection
+ Ability to pass CORI, SORI, fingerprinting, and other required background checks
Preferred Qualifications:
+ Prior supervisory or leadership experience
+ Home health nursing background
+ EMR experience (Icentrix strongly preferred)
+ Strong triage and medication management skills
+ Experience coordinating interdisciplinary care teams

Work Environment This is an onsite position based in North Adams, MA, with a Monday-Friday schedule. While hours are consistent, some flexibility is required to meet member and program needs.
You'll work with an experienced, dedicated team focused on delivering high-quality, person-centered care.

The pay range for this position is $80000.00 - $90000.00/yr.
