Pay: $110.00 per hour
$2000 SIGN-ON-BONUS-PRN
$7500 SIGN-ON-BONUS-FT

The Nurse Practitioner (NP) brings expert care to members, through clinical assessments, screenings, and education. This role may provide care in a variety of settings including member/participant homes, assisted living facilities, and skilled nursing facilities.
Essential Job Functions- Perform screenings including medical history, physical exam, medication reconciliation and education.
- Effectively identify, evaluate, and address the concerns of the member, employee or participant.
- Order and interpret diagnostic imaging, laboratory results and provide clinically appropriate interventions, additional tests, or referral as needed.
- Administer and order point of care and lab based testing
- Refer members to PCP, case management, disease management programs, and/or appropriate specialist as necessary.
- Work collaboratively with a variety of stakeholders including risk & quality teams, staffing, operations, physicians, case managers, social workers, care givers and ancillary medical personnel.
- Participate in all mandatory compliance and other training programs, seek guidance for compliance related concerns and adhere to all applicable laws, regulations and Matrix policies and procedures.
- Create and maintain clear documentation according to clinical standards, Matrix guidelines and protocols.
- Maintains positive control of all PHI information and compliance with OSHA and State Board of Nursing.
- May act as a preceptor for new providers.
- Other duties may be assigned
Educational Requirements:
- Bachelor of Science in Nursing preferred.
- Master’s Degree required OR commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority.
- Board certification by AANP, AACN or ANCC, in Adult, Family Practice Geriatrics or Adult-Gerontology Primary Care, or ACNPC-AG (Adult Gero) Acute Care NP, or ACNPC (Adult) Acute Care NP
- Current CPR certification.
- Multistate licensure preferred.

- Strong clinical and communication skills with the ability to work autonomously and collaboratively.
- Ability to manage multiple priorities and meet deadlines.
