What You'll Do

As an RN Nurse Supervisor, you will:

* Oversee healthcare services, clinical interventions, and ongoing medical needs for program members

* Participate in Individual Support Plans (ISPs) and interdisciplinary case conferences

* Coordinate medical care with primary care providers and external partners

* Advocate for the health, safety, and well-being of each program member

* Conduct pre-admission reviews, assessments, and develop AFC plans of care

* Ensure regulatory compliance and maintain accurate, confidential medical records

* Recruit, train, supervise, and support AFC caregivers

* Perform quality assurance activities, including audits and policy reviews

* Collaborate with multiple agencies to ensure coordinated, high-quality care

* Maintain all required certifications and follow safety protocols

What You Bring

Required Qualifications:

* Active RN license in the state of Massachusetts

* Minimum of 2 years of recent, hands-on experience with elderly or disabled adults

* Experience working within AFC or DDS programs

* Strong communication, organization, and documentation skills

* Ability to educate and train adult learners

* Basic computer proficiency and comfort using Zoom and EMR systems

* Valid driver's license, reliable personal vehicle, current insurance, and annual vehicle inspection

* Ability to pass CORI, SORI, fingerprinting, and other required background checks

Preferred Qualifications:

* Prior supervisory or leadership experience

* Home health nursing background

* EMR experience (Icentrix strongly preferred)

* Strong triage and medication management skills

* Experience coordinating interdisciplinary care teams

Work Environment

This is an onsite position based in North Adams, MA, with a Monday-Friday schedule. While hours are consistent, some flexibility is required to meet member and program needs.

You'll work with an experienced, dedicated team focused on delivering high-quality, person-centered care.

Pay and Benefits

The pay range for this position is $80000.00 - $90000.00/yr.
