Hospice RN – UHC Staffing Hospice RN

Location: Moab, UT

Start Date: 02-16-2026

End Date: 05-18-2026

Contract Type: Travel Contract

Shift Type: Day shifts (specific start/end times not provided);
On-call rotation 1-3 shifts per week (includes 3 weekends during the 13-week contract); must be able to respond within 45 minutes when on-call; holiday availability required.

Hours per Week: 32

Candidate Pay Rate: $63 per hour

Roles & Responsibilities
  • Perform comprehensive patient assessments and coordinate individualized hospice care plans
  • Provide expert pain and symptom management consistent with each patient’s plan of care
  • Educate and support patients and families on disease progression, comfort measures, and hospice philosophy
  • Collaborate closely with physicians, social workers, chaplains, volunteers and the interdisciplinary team
  • Participate in the on-call rotation and respond promptly to patient needs when assigned
  • Maintain accurate, timely clinical documentation in compliance with state, federal, and organizational policies
  • Deliver compassionate, dignified care in patients’ homes and serve as a trusted clinical resource for families
Qualifications
  • Current Registered Nurse (RN) license in Utah or a compact state
  • Minimum of 3 years of nursing experience (hospice, home health or palliative care experience preferred)
  • Current BLS certification required
  • Valid driver’s license and reliable transportation required for home visits
  • Able to respond within 45 minutes when on call
  • Willingness to work holidays and participate in weekend coverage as scheduled
  • Strong clinical assessment, communication, interpersonal and organizational skills
  • Compassionate demeanor and ability to support patients and families through end-of-life care
Education
  • Active Registered Nurse (RN) licensure in Utah or compact state
  • Current BLS certification
Knowledge & Skills
  • Patient assessment and care plan coordination
  • Pain and symptom management in hospice settings
  • Clinical documentation and use of hospice EMRs (Hospicesoft, Careficient preferred)
  • Interdisciplinary collaboration with physicians, social workers, chaplains and volunteers
  • Patient and family education on end-of-life care and self-care techniques
  • On-call triage and rapid response skills for home-based hospice patients
