Role SummaryOur client is seeking an experienced Licensed Practical/Vocational Nurse (LPN/LVN) to join a remote care management team supporting members enrolled in a regional health plan. In this role, you will guide individuals and families through complex health situations by coordinating care, supporting disease management, and helping members navigate the healthcare system effectively.
You will work with individuals facing acute conditions, long-term chronic illnesses, and end-of-life care needs. Your goal is to improve health outcomes, support informed decision-making, and ensure members receive appropriate, timely, and well-coordinated care.
Key Responsibilities- Deliver care coordination and case management services to health plan members
- Support disease management initiatives through education, coaching, and engagement
- Assist members during care transitions, including hospital discharges and referrals
- Collaborate with physicians and interdisciplinary care teams to align treatment plans
- Advocate for members to ensure access to appropriate services and resources
- Provide guidance to members and families facing complex or end-of-life care situations
- Promote effective use of healthcare benefits and community resources
- Completion of an accredited practical nursing program
- Associate degree in nursing, healthcare, or a related discipline
- Active Oregon LPN license at time of hire
- Willingness to obtain additional state licensure if required
- Valid driver’s license and auto insurance (travel may be required on occasion)
- Minimum of 5 years of hands-on clinical nursing experience
- At least 2 years of experience collaborating with physicians in managing patient care
- Bachelor’s degree in health education or a healthcare-related field
- Background in care management, care coordination, or care navigation
- Familiarity with health insurance operations and regulatory environments
- Experience working with public or private healthcare oversight organizations
Oregon-based salary ranges (depending on location):
- Non-metropolitan areas: approximately $33 – $51 per hour
- Portland metro area: approximately $32 – $55 per hour
Final compensation is determined based on experience, skills, and other job-related factors. Additional pay opportunities may be available, such as overtime, shift differentials, or incentive programs.
