RequiredSkills
- Valid Nurse Practitioner (NP) license with palliative care certification preferred
- Minimum of 2 years clinical experience, with pediatrics experience highly desirable
- Strong knowledge of hospice regulations and Medicare recertification procedures
- Excellent communication and interpersonal skills
- Ability to work independently and collaborate with multidisciplinary teams
- Comfortable conducting face‑to‑face assessments and providing patient education
- Previous experience in a hospice or palliative care setting
- Experience with home visits and inpatient rounding
- Bilingual skills or additional language proficiency
Education and Experience
- Master's degree in Nursing or related healthcare field
- Proven track record in pediatric palliative care and hospice services
- Prior experience working in home health or outpatient environments
- Valid driver s license and reliable transportation for home visits
- Ability to travel to patient locations within Fresno and surrounding areas
- Certifications in CPR and other relevant credentials
- Availability for 1-2 days per week, with schedule flexibility to accommodate full or half days
- Willingness to work in varied settings, including homes, SNFs, or inpatient units
