Position: Registered Nurse (HUD/VASH)
The VA Greater Los Angeles Healthcare System is seeking highly skilled and experienced Registered Nurses for the HUD-VASH Community based program. Registered Nurses perform visits and provide care to veterans within the community at least 3 to 5 days a week. Must be comfortable to be in the community or Transitional housing environments where veterans reside. Driving is mandatory.

The Housing and Urban Development Veterans Affairs Supportive Housing (HUD/VASH) Registered Nurse (RN) focus is on individual growth and development in practice with increased levels of self-direction. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.

This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems. Coordinates care with

multiple providers across all levels and sites of care.

The HUD/VASH RN develops a plan of care using information from the patient, family, and provider. Bridges the gap between discharge and primary care follow-up. Identifies high-risk patient population and transitions to appropriate discipline (i.e., Primary Care, Home Based Primary Care, Hospice/Palliative Care etc.). Responsible for intensive monitoring, follow-up, and clinical management of high-risk patients. Collaborates with interdisciplinary team members to develop and implement the Veteran Housing Stability Plan.

Contributes to the team-based assessment of the ability of individual Veterans to live independently in the community, as part of eligibility determination for Veterans with complex medical and psychological risk factors. Provide recovery-oriented and Housing First services, with the goal of establishing the Veteran independently in the community, at the Veteran's highest level of functioning. The HUD/VASH RN addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice.

Evaluates care and outcomes to ensure timely and appropriate provision of services. The nurse brings principles and theory with limited application of critical thinking skills.

Job duties include, but are not limited to, the following:

* Provides chronic health assessment to include, weight, vital signs, allergies and food securities.

* Optimizes primary care engagement of HUD-VASH Team's veterans.

* Provides patient education in the areas of disease self-care, identification of urgent needs, educational training in activities of daily living, medication administration, wound management, and relational challenges.

* Communicates with the interdisciplinary team to coordinate complex care and inpatient/ED discharges to improve adherence to treatment plan.

* Supports preventive care interventions for annual vaccinations, substance cessation, sobriety, nutrition, and exercise/activity.

* Ensures clinical reminders are completed and utilizes functional assessment tools to asses veteran level of ADL's and care needs.

* Completes ordered tests, immunizations, patient education based on clinical reminders. Vital signs, weight and clinical reminders to be completed with each home visit. Ensure equipment is clean and in good working order.

* Ongoing documentation and daily communication via Cell phone, Skype and/ or e-mail, to the VASH team and/or supervisor especially when in the community.
