Position: Resident Care Manager and Registered Nurse Hybrid Role

Description

The primary role of the Registered Nurse is to supervise staff, monitor patients, implement treatments, develop patient care plans for long-term needs, administer medications, follow-up with families and provide the highest quality of care to all residents/patients. Provides clinical care in compliance with current federal and state guidelines, and company regulations.

Position Summary

All employees of Cascadia Healthcare are required to submit and be cleared to work in the facility per each state’s specific background check requirements prior to contact with patients/residents.

Resident Care Manager (RN) Overview

The primary purpose of the Resident Care Manager (RN) is to develop, evaluate, implement health care plans for individual patients, assist with management of individual medical plans, while providing the highest level of patient care. The Resident Care Manager (RN) also communicates treatment plans between families, doctors, providers and residents/patients.

This would be a hybrid role of working as a floor nurse a few days a week as well as an RCM a few days a week.

Essential Functions
  • Provides the highest quality of patient care to facility residents/patients.
  • Documents care performed and observations of resident status in the clinical record promptly by utilizing standard clinical documentation guidelines as required by the facility, and local, state and federal rules and regulations.
  • Communicates with resident’s physician.
  • Completes all nursing documentation: assessments, orders, progress notes, flow sheets, consents, and any assigned tasks and ensures that said documentation accurately reflects services required by the resident and provided to the residents.
  • Participates in medication management to promote optimal safety and effectiveness for residents by:
    • Administering medications as ordered, including next scheduled dose following admission/readmission. Notifies Supervisor of medications not available;
    • Preparing, administering and documenting medications as prescribed;
    • Observing and reporting resident responses to medication;
    • Identifying and promptly communicating adverse drug reactions;
    • Initiating enteral or parenteral therapy as ordered;
    • Maintaining narcotic records accurately;
    • Ordering or arranging for ordering of pharmaceuticals;
    • Notifying physicians of automatic stop order; and/or
    • Completing recapitulated physician orders review, as assigned.
  • Assists with or institutes emergency measures for sudden adverse developments with residents. Directs and administers cardiopulmonary resuscitation.
  • Punctuality and regular attendance for assigned shifts.
  • Evaluates updates resident/patient health care plans to achieve person-centered care.
  • Conducts assessments for new residents/patients.
  • Actively participates in clinical meetings.
  • Collaborates with other members of the health care team, as needed.
  • Utilizes the electronic medical record to establish & validate parameters are established as indicated.
  • Establishes, monitors, and documents person-centered care, as indicated.
  • Uses SBAR to communicate patient’s change of condition with physician.
  • Reviews test results from medical exams.
  • Implements physician orders and follow up with treatment plans.
  • Communicates with families, health providers and patients. Receives consent, as required.
  • Documents in the medical record to clearly reflect patient care & current medical condition.
  • Ensures completeness of medical records.
  • Provides emotional support to families and patients.
  • Provides education about health care plans to patients and families.
  • Collaborates with other clinical staff to successfully implement patient plans of care & provides excellent customer service.
Other Functions
  • Ensures compliance with company’s clinical policies and procedures.
  • Follows the guidelines established from the Nurse Practice Act, company standards, policies and procedures and sound nursing judgment.
  • Performs other related duties, as assigned.
  • Obtains accurate information from physicians, residents/patients, and payor source(s) regarding the expected discharge plan and communicate this information to the interdisciplinary team at the facility.
