Hospice - Palliative Nurse Practitioner (Part-Time, Home-Based with Travel)

This innovative, flexible role offers the opportunity to provide compassionate, patient-centered care in a home-based setting while supporting our medical team with vital evaluations and education. Key Responsibilities:
  • Perform face-to-face recertification evaluations for Medicare patients in collaboration with the Medical Director
  • Conduct home visits to patients in nursing facilities, care facilities, or private residences
  • Support inpatient rounding and participate in clinical team education as needed
  • Communicate effectively with care managers to coordinate follow-up and ongoing patient care
  • Assist in providing compassionate, high-quality palliative care aligned with patient wishes and medical directives
Required Skills:
  • Active Nurse Practitioner (NP) license
  • Proven experience in palliative care, with preference for pediatrics experience
  • Strong clinical assessment and evaluation skills
  • Excellent communication and teamwork abilities
  • Ability to work independently and manage flexible schedules
Nice to Have

Skills:
  • Experience with Medicare recertification processes
  • Prior home health or hospice care experience
  • Familiarity with electronic medical records (EMR) systems
  • Additional certifications in hospice, palliative care, or related fields
Preferred

Education and Experience:
  • Master’s degree or higher in Nursing (MSN, DNP)
  • Minimum of 2 years clinical experience in palliative or hospice care
  • Pediatric care experience strongly preferred
Other Requirements:
  • Valid driver’s license and reliable transportation for home visits
  • Ability to travel locally as needed; flexible schedule to accommodate various shifts (full or half days)
  • Willingness to work part-time, 1-2 days per week
  • Compliance with all relevant healthcare regulations and safety standards
