Job OverviewDaVita Integrated Kidney Care (IKC) is looking for a Full‑Time Nurse Practitioner (NP) to join us in the Florence, AL region.
As an IKC NP you will support some of our most complex patients, assisting them in navigating a challenging healthcare system while delivering holistic, integrated care to improve the quality of life for our kidney patients. You will work autonomously and in collaboration with all healthcare team members to coordinate and facilitate quality, consistent, cost‑effective care.
Key Responsibilities- Complete comprehensive health assessments.
- Manage complex patients with a focus on reducing hospitalizations and improving health outcomes.
- Work a Monday‑to‑Friday schedule; be flexible to accommodate patients and nephrology partners.
- Operate on a full‑time, self‑managed schedule as a hybrid‑remote clinician, traveling within an assigned geography to Dialysis Clinics and Nephrology Practices.
- No nights, weekends, or on‑call shifts.
- Provide patient‑centered care within a value‑based, person‑centered, comprehensive kidney‑care model.
- Passion for delivering care to patients with complex, chronic illnesses.
- Minimum of two (2) years’ experience as a Nurse Practitioner (NP).
- Current unrestricted NP license to practice in the state.
- Current DEA license required or eligibility to obtain within 90 days of hire.
- Current CPR or BLS certification.
- Intermediate computer skills and proficiency in MS Word, Excel, & Outlook.
- Home office with high‑speed internet and reliable transportation required.
- Experience in value‑based care or population health, including documenting and coding from a Medicare Advantage risk perspective and conducting Health Risk Assessments on high‑risk/chronically ill patients.
- 5+ years’ experience as a Family NP, Primary Care NP, or experience in Internal Medicine, Cardiology, Nephrology, or Case Management.
- Spanish‑speaking preferred in some markets.
- Experience managing chronic diseases (CHF, diabetes, hypertension).
- Experience managing population health (hospitalization metrics).
- Competitive salary range: $85,000 – $135,000 per year.
