Access Healthcare Associates is hiring a Nurse Practitioner (NP) or Physician Assistant (PA) to join our mobile primary care team. This role is ideal for advanced practice providers seeking autonomy, flexibility, and meaningful patient connections?outside of the traditional clinic setting.
Since 2007, Access Healthcare Associates has been a physician-led mobile medical practice providing high-quality home-based primary care to seniors. Our fee-for-service care model focuses on delivering relationship-driven, comprehensive care directly where patients live?whether in private residences, assisted living communities, or skilled nursing facilities.
Key ResponsibilitiesConduct in-home visits to provide primary care services in residential and community-based settings.
Complete detailed health assessments, manage chronic and acute conditions, and develop personalized care plans.
Coordinate with specialists, ancillary services, and caregivers to ensure well-rounded, patient-centered care.
Document patient encounters using a secure EMR system.
Operate independently while supported by a strong clinical and administrative leadership team.
Active and unrestricted NP or PA license.
Board certification (AANP, ANCC, or NCCPA).
Experience in geriatrics, internal medicine, family practice, or home-based care preferred.
Valid driver's license and reliable transportation.
Strong clinical judgment, time management, and professionalism.
Commitment to delivering compassionate, high-quality care in home environments.
Fee-for-service model with total annual earning potential of $150,000 ? $250,000+.
