Registered Nurse Assessor- Bronx

** Registered Nurse
** to join our team in the Bronx. Help New York residents achieve a better quality of living by conducting UAS assessments and providing leadership to our regional team of Nurse Evaluators.

** The Basics*

** Pay Rate:**  $60/hr

** Bronx, NY (Regional field travel + Remote/Home Office)

* Hours:

** Mon-Fri (8am-6pm) | Sat-Sun (10am-6pm)  _Schedule flexibility available. 37.5 hours per week._

** Key Responsibilities*
* +  
** Field Evaluations:
** Perform in-home patient evaluations in the Bronx area focusing on health strengths and care preferences via the  
** UAS-NY tool** .

+  
** Team Support:
** Act as a lead by instructing and observing new Nurse Evaluators during assessment visits to ensure high-quality service.

+  
** Administrative Oversight:
** Review consumer health referral forms and documentation for accuracy; transmit data into electronic forms promptly.

+  
** Community Impact:
** Facilitate enrollment into managed care health plans to ensure patients receive the support they need.

** Qualifications*
* +  
** Licensure:
** Valid RN license in the state of New York.

+  
*
* Experience:

** 2+ years of RN experience in home care.

+  
** Technical

Skills:

** Prior experience using the  
** UAS-NY Community assessment tool** .

+  
** Travel:
** Ability to travel locally within the Bronx and surrounding area up to 50%.

** Home Office:
** Reliable high-speed internet (min 20 Mbps download) and a personal cell phone.
