• Conduct in‑home client assessments
• Develop individualized plans of care
• Provide nursing interventions focused on realistic client outcomes
• Update care plans based on client progress
• Maintain accurate clinical documentation
• Support caregiver onboarding, skills assessments, drug testing, and training
• Perform additional clinical tasks as needed

• Current RN license
• Graduate of an accredited nursing program
• CPR certification
• Reliable transportation
• Strong communication and documentation skills
• Geriatric or home care experience preferred
