Mecklenburg County & Wake County, North Carolina Nurse Practitioner – Home Health Assessments

Rhino Medical is seeking experienced Nurse Practitioners for a home-based health assessment opportunity in Mecklenburg County and Wake County, North Carolina.

This is a flexible, weekday-only position focused on annual risk assessments and chronic condition reviews conducted in patients’ homes. This role is strictly assessment-based — no prescribing, treatment management, or ongoing primary care panel responsibilities required.

Position Details
Location Options:

• Mecklenburg County, NC
• Wake County, NC

Schedule Options:

• Full-Time: 40 hours/week
• Part-Time: 20 hours/week

Hours:

• Monday–Friday
• 8:00 AM – 5:00 PM
• 1-hour lunch break included

Schedules are released 2–3 weeks in advance.
Providers are paid for a full scheduled day regardless of patient volume, provided availability is properly entered into the scheduling system.

Role Responsibilities

• Conduct 5–6 in-home health assessments per day
• Perform annual risk assessments
• Review and document chronic conditions
• Conduct A1C testing when applicable
• Escalate urgent findings to the patient’s primary care provider

This is a non-treatment, non-prescribing assessment role.

Compensation & Support

• Paid for full scheduled day
• Mileage reimbursed at IRS standard rate
• 1–3 hours of paid orientation
• Ongoing education modules completed during downtime within working hours

Candidate Requirements

• Active North Carolina Nurse Practitioner license
• Must be enrolled and credentialed with Medicaid (confirmation required at submission)
• Prior clinical experience preferred
• Comfortable working independently in patient home settings
