Overview
Jethro Health is seeking an experienced Home Health Agency RN/LPN to see patients in the North Shore area:
Lynn, Peabody, Nahant, Revere, Winthrop, East Boston, Chelsea, Salem, Swampscott, Wakefield, Reading, Woburn, Saugus.
We seek a compassionate and dedicated LPN or RN to join our Home Health team. The ideal candidate will provide quality care to patients in their homes, ensuring their well-being and promoting independence.
This is a Per-Diem or Part Time position, with the potential to become Full-Time.
Responsibilities- Conduct thorough assessments of patients health status and needs in their home environment.
- Develop and implement individualized care plans based on patient assessments and medical history.
- Provide education and support to patients and their families regarding health conditions, medications, and self-care techniques.
- Assist with discharge planning from hospitals or rehabilitation facilities, ensuring a smooth transition back to home care.
- Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
- Collaborate with interdisciplinary teams, including physicians, social workers, and therapists, to coordinate comprehensive care.
- Document all patient interactions accurately in compliance with ICD-10 coding standards.
- Offer health coaching to empower patients in managing chronic conditions effectively.
- Provide specialized services such as esthetic laser treatment when applicable.
- Support family planning initiatives and educate patients on related health topics.
- Strong clinical skills with a focus on acute care and senior care practices.
- Excellent communication skills for effective patient education and collaboration with healthcare teams.
- Proficient in discharge planning processes and the ability to navigate complex healthcare systems.
- Knowledge of ICD-10 coding for accurate documentation of patient encounters.
- Experience working in a teaching hospital environment is preferred but not required.
- Ability to provide compassionate care while maintaining professional boundaries.
- Strong organizational skills to manage multiple patient cases effectively.
- Commitment to ongoing professional development and staying current with best practices in nursing care.

Job Types: Full-time, Part-time, Per diem
Pay- RN - Oasis Start of Care: $100 per visit
- RN - Recertification: $60 per visit
- RN - Oasis Resumption of Care: $80 per visit
- RN - Skilled Nursing visit: $45 per visit
- LPN - Skilled Nursing visit: $40 per visit
- Pay: $37.73 - $45.00 per hour
