Part time hourly quality coordinator RN position will support ambulatory projects related to safety, regulatory, accreditation, and quality, reporting to M. Colette Carver.

Performing under the direct supervision of the Home Health/Hospice manager of Quality and Patient Safety, the Quality Coordinator incorporates evidenced based practice, supported by industry benchmarking, in developing nontraditional skills required or successful management of home-based patients; collects, synthesizes and interprets staff feedback and performance improvement data for both the home health and hospice programs in all locations; creates and maintains staff development and performance improvement monitoring activities to ensure program integrity and regulatory compliance;

coordinates orientation and on-going staff education as determined by individual and group needs assessments; supports chronic disease management/telehealth program; participates in the evaluation of new policies and protocols and changes to current policies and protocols, presents findings to quality committees, staff, and HH/HO leadership as appropriate.

Key Responsibilities
  • Integrates Evidence Based Practice and agency benchmarks to develop non-traditional skills for successful management of home-based patients.
  • Collects, synthesizes and interprets performance improvement data for home heal h and hospice
  • Collaborates with ongoing accreditation readiness and performance improvement projects and teams as needed
  • Develops and maintains performance improvement monitoring activities to determine individual and group training needs as they arise, communicating a plan of action to the immediate supervisor and any needed educators in a timely manner, formally tracking progress
  • Consults with others, formally and informally, regarding organizational or process change, solutions to problems and/or barriers.
  • Maintains, updates and provides appropriate reports from data sources.
  • Verifies and provides data to the Manager of Quality and Patient Safety as required by regulatory needs or as specified in the Quality Improvement plan.
  • Independently assists in the development, design and production of spreadsheets.

    - completing assignments with minimal supervision and direction.
  • Serves as an educational resource to others in the department for projects and process management education,
  • Coordinates resources/opportunities to ensure compliance with staff competencies and/or identified needs
  • Supports the Telehealth program as needed
  • Participates in an annual review of clinical protocols and policies, submitting proposed changes to HH/HO Leadership as appropriate.
  • Coordinates the development and provision of annual skills competency training, protocol testing, initial training for new medical equipment and/or procedures. Maintains records of all.
  • Coordinates the orientation of all new employees of the Home Health/Hospice Departments. Ensuring appropriate scheduling of live and on-line classes, developing and following up on employee goals.
  • Identifies and revises orientation and continuing education requirements to maintain compliance with all accrediting, regulating, and credentialing agencies in conjunction with the HH/HO Leadership Team.
What We Require

Education: Bachelor degree required.

Experience: Must have a minimum of 3 to 5 years of Home Health and/or Hospice experience.

Licensure, certification, and/or registration: Registered Nurse in State of Virginia.
