Alanté Health is seeking a dedicated Nurse Practitioner to redefine the future of home-based primary care in Arizona. In this hybrid role, the Nurse Practitioner will provide comprehensive patient care through home care and telehealth.

We provide Chronic, Principal, Transitional and Wellness Care Management for seniors that improves patient outcomes while significantly reducing hospital readmissions and overall healthcare costs. We go to them, whether they are at their own home, in Assisted Living or a Skilled Nursing Facility;

Responsibilities
  • Perform comprehensive patient assessments and collaborate with facility staff on preventive services and early changes in condition recognition.
  • Manage acute and chronic conditions, including medication adjustments.
  • Order and interpret diagnostic tests.
  • Develop, implement, and modify treatment plans as needed.
  • Participate in quality assurance and improvement activities.
  • Provide clinical education and training for patients, families, and facility staff.
  • Adhere to state-specific NP scope of practice requirements.
  • Demonstrate knowledge of etiologies, risk factors, pathophysiology, and presentation of acute and chronic conditions.
  • Travel Requirement

    • This role includes in-person patient visits within approximately a 25-mile radius of the clinician’s hiring home address, or as otherwise assigned based on business needs.
    • Travel time during the workday is considered part of the role’s responsibilities and will follow all applicable wage and hour guidelines.
    • Clinicians are expected to maintain a valid driver’s license, reliable transportation, and the ability to meet scheduling requirements for in-home visits.
  • Geographic Assignment

    • Service areas may be adjusted based on patient volume, care coordination needs, and operational demands.
    • Travel expectations apply to all Nurse Practitioners hired under this updated job description.
    • Existing Nurse Practitioners will maintain their originally assigned geographic expectations as part of a grandfathered arrangement, unless they voluntarily opt into the updated travel model.
Qualifications
  • Master’s degree in Nursing or RN license.
  • Advanced Practice Nursing license.
  • NP Board Certification from recognized certifying body.
  • Credentialed with insurance companies preferred.
  • Fingerprint clearance.
  • Unrestricted prescribing and dispensing privileges with DEA licensure required.
  • Driving is a requirement for this position. Current state driver's license and proof of insurance is required.
  • 1 to 2 years' NP experience preferred.
  • Experience with adult, geriatric populations and population health management preferred.
  • Strong communication, organizational, and customer service skills.
  • Knowledge of Skilled Nursing/Transition Rehabilitation Centers (preferred).
  • Ability to handle a high volume of patients with skill and confidence.
  • Knowledge and use of an Electronic Health Record.
  • Ability to work with confidential information including HIPAA protected information.
  • Ability to be proactive and initiate processes with little to no supervision.
