About the RoleThe Clinical Support Nurse (CSN) coordinates patient care and admission service as it relates to home infusion therapies, including collaboration and coordination between the home infusion pharmacy, hospital/facility case management, home health agencies, physician offices, and managed care providers for patients in or returning to the home setting. The Clinical Support Nurse is a cross functional nursing position that supports both the clinical and sales and marketing objectives of Palmetto Infusion Services.
Schedule:
Monday - Friday; 8:00 AM – 5:00 PM, on call as required.
Service Areas:
Myrtle Beach, SC and surrounding areas.
- Graduate from an accredited school of nursing.
- Active Registered Nurse license in the state(s) of practice.
- 2 years of RN experience.
- BLS or obtain upon hire.
- Proficient at partner programs (i.e.: MS Outlook, Word, Excel).
- Ability to cope with ambiguous, changing environment while under pressure in a calm
- Must be able to work as a team member and develop productive and cooperative working relationships with all members of the Palmetto Infusion team.
- Provides infusion therapy and other skilled nursing care to patients in both the clinical and home environment including, but not limited to:
- Patient assessment and reassessment
- Patient and caregiver education/teaching
- Development and revision of the Plan of Care and the Plan of Treatment
- Implementation of physician orders
- Monitoring patient response
- Response to emergency patient needs
- Psychosocial support
- Provides pre-discharge patient assessments when appropriate.
- Communicates effectively with clinical, reimbursement, operational, and management personnel as required to coordinate home infusion referrals.
- Works collaboratively with providers, patients, and caregivers to establish priorities and set obtainable and measurable goals of patient care.
- Provides support and coordinated marketing efforts with sales team to include, but not limited to, providing educational programs to referral sources as requested.
- Completes and submits all required patient care documentation and reports in an accurate and timely manner.
- Effectively communicates changes in patient condition to care team, physician, and to Supervisor, as appropriate.
- Maintains knowledge and proficiency in evidence-based best practices through continuing education, staff meetings, and in-service opportunities.
- Participates in the orientation of other staff as requested.
- Occasional travel may be required.
- Takes on-call responsibility as assigned to provide 24-hour availability according to protocols.
