Supportive Care Case Manager RN - REMOTE

• Support complex patients with CKD/ESKD by identifying their medical, social, emotional, and financial needs
• Collaborate with healthcare team members to coordinate quality care and facilitate effective treatment plans
• Educate patients on treatment options and ensure care aligns with their preferences and goals
• Facilitate communication between patients, families, and the care team
• Develop and maintain relationships with supportive care providers and coordinate transitions of care

• Minimum of 2+ years experience as an RN in Hospice or Palliative medicine
• Minimum of five (5) years experience as an RN
• Current RN License required; BSN preferred
• Current CPR or BLS certification
• Intermediate computer skills and proficiency in MS Word, Excel & Outlook

• Experience as a DaVita IKC RN Case Manager
• Living in the Mountain or Pacific Time zones
• 2+ years of Case Management or Chronic Care Management experience
• Minimum of two (2) years experience in renal nursing
• Spanish-speaking skills
