• Assess patients' medical problems and complaints by phone and in person.
• Provide information and advice for home treatment when indicated and arrange evaluation by a provider when needed—by scheduling an appointment or telephone follow‑up.
• Review critical lab results daily and work within the care team for appropriate and timely follow‑up provider consultation.
• Perform vaccine administration, medication administration, diagnostic tests and procedures as necessary.
• Educate patients and families by phone and at appointments about minor illnesses, medication usage, health maintenance needs, blood sugar monitoring, nebulizer usage, and other prescribed health‑care regimens.

• Massachusetts RN license
• Previous practice experience
