Supportive Care Case Manager RN - REMOTE

• Support complex patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD) by navigating the healthcare system
• Collaborate with healthcare team members to coordinate and facilitate quality, cost-effective care
• Educate patients on treatment options and ensure care aligns with their preferences and goals
• Develop and update advance care plans in partnership with patients and families
• Facilitate communication between patients, families, and the care team to ensure streamlined care transitions

• Minimum of 2+ years experience as an RN in Hospice or Palliative medicine
• Minimum of five (5) years experience as an RN
• Current RN License required; BSN preferred
• Current CPR or BLS certification
• Intermediate computer skills and proficiency in MS Word, Excel, and Outlook

• Experience as a DaVita Integrated Kidney Care RN Case Manager
• Living in the Mountain or Pacific Time zones
• 2+ years of Case Management or Chronic Care Management experience
• Minimum of two (2) years experience in renal nursing
• Spanish-speaking capabilities
