- Support complex patients in navigating the healthcare system through assessment of medical, social, emotional, and financial needs
- Collaborate with healthcare team members to coordinate patient care and facilitate quality interventions
- Educate patients on treatment options and assist in developing care plans that align with their values and preferences
- Facilitate communication between patients, families, and the care team to ensure seamless care transitions
- Provide guidance to nursing peers in comfort care management when palliative options are not available
- Minimum of 2+ years experience as a Registered Nurse in Hospice or Palliative medicine
- Minimum of five (5) years experience as a Registered Nurse
- Current RN License is required; BSN preferred
- Current CPR or BLS certification
- Intermediate computer skills and proficiency in MS Word, Excel, and Outlook
- Experience as a DaVita Integrated Kidney Care RN Case Manager
- Residing in the Mountain or Pacific Time zones
- 2+ years of experience in Case Management or Chronic Care Management
- Minimum of two (2) years experience in renal nursing
- Spanish-speaking proficiency
