We are seeking a Nurse Practitioner or Physician Assistant to join our mobile, primary care practice in a hybrid role
. This position includes mobile clinic visits throughout the Tri Cities, WA area and flexibility for some remote work
.

This is an excellent opportunity for a provider who enjoys autonomy, meaningful patient relationships, and caring for vulnerable populations.

• Perform patient assessments and develop individualized care plans
• Diagnose and manage acute and chronic medical conditions
• Provide in‑home mobile visits to primarily elderly and medically fragile patients
• Deliver primary care and other services such as wound management, foot care, memory care, and limited mental health support
• Document encounters in Athena EMR
• Collaborate closely with our physician and clinical team
• Support patient education, follow‑up, and ongoing case management

• Active Washington State NP or PA license and valid DEA
• Primary care or specialty clinic experience preferred
• Strong communication and relationship‑building skills
• Comfortable working independently in a hybrid/mobile model
• EMR proficiency (Athena experience is a plus)
• Commitment to quality, integrity, and regulatory compliance

• Productivity‑based pay with competitive earning potential
