Requirements:

Active Registered Nurse license

Knowledge of hospice care, Medicare, and Medicaid guidelines.

Strong assessment, documentation, and communication skills.

Ability to work independently in home-care environments.

Experience with IV therapy and venipuncture preferred.

Valid driver’s license, reliable vehicle, and insurance.

Ability to meet physical demands of home-based patient care.

Commitment to hospice philosophy and ethical nursing standards.

Responsibilities:

Respond to after-hours patient/family calls and assess needs.

Provide pain and symptom management per physician plan of care.

Perform ongoing patient assessments and document care accurately.

Communicate with physicians and interdisciplinary team members.

Educate and support patients and caregivers.

Coordinate referrals to hospice disciplines and community resources.

Maintain confidentiality and comply with agency policies.

Work independently and prioritize urgent situations.
