* What You’ll Do:

*** Completes an initial, comprehensive and ongoing assessments of patient and family to determine home health or hospice needs.
* Provide respectful, compassionate care in patients’ homes.
* Develop patient focused care plans that establish goals based on nursing diagnoses, and incorporate rehabilitative nursing actions.
* Collaborate with interdisciplinary team members.

* What You Bring:

*** Active nursing certification (state-licensed).
* Valid driver’s license, reliable transportation, and current auto insurance.
* Strong communication skills and a team-oriented mindset
* Ability to work independently in home settings
* Commitment to patient-focused care
* Current CPR Certification
