Schedule:
Home-based patient visits with coordinated scheduling
Highlights:
Provide comprehensive evaluation and care for home health patients including initial, resumption, and recertification assessments
Coordinate multidisciplinary care teams including RNs, LPNs, therapists, social workers, and home health aides
Develop and implement individualized care plans in collaboration with physicians, patients, and families
Deliver age-appropriate interventions addressing physical, emotional, developmental, psychosocial, and educational needs
Manage patient episodes from admission through discharge with therapeutic, preventative, and rehabilitative focus
Enjoy the autonomy and flexibility of home-based nursing care
Requirements:
Current RN license in Georgia
Home health nursing experience preferred
Strong assessment and care coordination skills
Excellent communication and collaboration abilities across multidisciplinary teams
Valid driver's license and reliable transportation for home visits
