Overview
Join our healthcare team as a part-time Home Health Nurse Practitioner, delivering compassionate care directly in patients' homes.
Responsibilities- Conduct comprehensive assessments and develop personalized care plans in collaboration with healthcare professionals.
- Administer treatments like IV infusions, medication management, and wound care.
- Perform procedures such as catheterization, phlebotomy, and basic life support to ensure patient safety.
- Educate patients on disease management, medications, and lifestyle modifications.
- Utilize EHR systems like Epic or eClinical
Works for accurate documentation. - Coordinate with multidisciplinary teams for seamless care and discharge planning.
Requirements include an active Nurse Practitioner license, experience in critical care preferred, proficiency with EMR/EHR platforms, and strong assessment and communication skills.
