Registered Nurse Care Manager

Help seniors stay healthier—by keeping care coordinated, proactive, and patient-centered. You’ll work alongside primary care providers and an interdisciplinary team to guide high-risk patients across inpatient, ambulatory, and post-acute settings—closing gaps, reducing avoidable utilization, and improving the care experience.

• Identify and enroll high-risk patients; complete whole-person assessments
• Build and monitor care plans + self-management plans with patients/caregivers and providers
• Coordinate transitions of care and follow-up across settings
• Close gaps in care (med rec, education, referrals, community resources)
• Document care plans and progress in the medical record/case management tools

• Active RN license (BSN preferred)
• Bachelor’s degree or clinical licensure/certification with 2–4 years’ experience
• Strong communication, organization, and comfort navigating complex patient needs

Hybrid: split time between our Dayton, Ohio office and remote work.
