Overview
Join our healthcare team as a Home Health Nurse Practitioner providing compassionate care directly in patients' homes.
Responsibilities- Conduct assessments, develop personalized care plans, and perform treatments including IV infusions, wound suturing, and medication management.
- Perform clinical procedures like catheterization, phlebotomy, and basic life support, ensuring patient safety.
- Educate patients on disease management, medications, and lifestyle modifications.
- Utilize EHR systems such as Epic or eClinical
Works for documentation. - Coordinate with multidisciplinary teams for care continuity and discharge planning.
- Valid Nurse Practitioner license
- Experience in critical care preferred
- Proficiency with telehealth and EMR/EHR systems
- Strong assessment skills
- Experience with diverse populations and complex procedures is a plus
