We’re seeking Primary Care NPs to provide on‑site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.

• Routine & urgent assessments; chronic disease management
• Post‑discharge follow‑ups, medication reconciliation, labs/diagnostics
• Coordinate with facility staff, specialists, families/POAs
• Same‑day documentation; CMS/regulatory compliance

Full‑time position with a structured weekday schedule. Spend 3 days delivering on‑site care across Stockton, Modesto, and Amador with strategically clustered routes designed to minimize travel and maximize time with patients. The remaining 2 days are remote, giving you flexibility to work from home for administrative and documentation tasks.

• Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
• Geriatrics or facility‑based experience preferred; EMR proficiency

• Annual compensation range: $150,000–$175,000 (based on experience, schedule, and patient volume)
• Additional earning opportunities based on coverage and panel needs
• W‑2 with full benefits available; mileage reimbursed per policy
