As a Nurse Assessor, you act as a crucial guide and advocate. Your primary responsibilities include:
- Comprehensive Assessments: Conducting medical and psychosocial home visits and evaluations to identify client needs and appropriate services.
- Care Plan Development: Creating personalized care plans to help clients access home and community-based support.
- Advocacy & Coordination: Championing client autonomy, coordinating resources, and documenting progress to inform the AERS program.
- Workload: Must complete a minimum of 17 assessments per week
. - Note: This is an investigative, charting role, not treatment-based.
- Licensure: Valid and active Maryland RN license
. - Experience: 2+ years of experience in medical-surgical, geriatric, and/or p case management
. - Travel: Must be willing to travel throughout Prince George County, MD
, and possess reliable transportation and a valid driver's license. - Tech
Skills:
Proficiency in navigating electronic medical records (EMR). - Commitment: Must be available full-time (M-F) and fully dedicated to this assignment (no secondary employment during work hours).
