• Provide direct patient care in a home setting, ensuring the comfort and well‑being of patients.
• Administer medications and treatments as prescribed by the physician (Wound Vac, IV, Certified, Wound care, etc.).
• Monitor and record vital signs, such as blood pressure, temperature, and heart rate.
• Implement plan of care initiated by the registered nurse / referring doctor.
• Educate patients and their families on proper healthcare procedures and disease management.
• Provide accurate and timely documentation consistent with the plan of care.
• Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST) to the registered nurse and/or Clinical Supervisor/Nursing Supervisor.
• Participate in personal and professional growth and development.
• Use equipment and supplies effectively and efficiently.

• Valid nursing license in the state of California.
• Minimum 1 year of experience as an LVN in Home Health.
• Proficient in taking and interpreting vital signs.
• Full time / part time position.
