Full Time RN Clinical Manager for Hospital at Home Program

Location:

Stuart to Vero Beach, FL (Hybrid)

Schedule:

Monday thru Friday, 8AM to 5PM

Salary: $72,000 annually plus Benefits

• Lead and mentor home health staff and aides
• Oversee patient care coordination and program operations
• Ensure clinical excellence and compliance with state and federal standards
• Collaborate with referral partners, physicians, and interdisciplinary teams
• Enjoy a consistent Monday to Friday schedule and supportive team environment

• Manage patient intake, authorizations, and care planning for skilled and non‑skilled services
• Conduct start‑of‑care and supervisory visits, as well as orientations and training for staff
• Provide clinical oversight aligned with best practices and agency policies
• Support and mentor field clinicians to ensure quality care delivery
• Maintain compliance with all regulatory and accreditation requirements

• Active RN license in Florida, required
• Minimum 1 year of home care experience, preferred
• Experience with Hospital at Home programs, highly preferred
• Valid driver’s license and reliable transportation
• Strong communication, leadership, and time‑management skills
• EMR proficiency;
  Alaya Care experience a plus
