Capital Caring Health in Lanham, Maryland seeks a dedicated RN, Case Manager to provide comprehensive patient and family care in their homes. This role includes assessing needs, coordinating care plans with an Interdisciplinary Team (IDT), and educating families on palliative care. Qualified candidates must have an active RN license in Maryland, at least one year of nursing experience, and excellent communication skills.

Salary range is $69,720.00 - $ based on experience.
