• Clinical Care Management: Conduct comprehensive patient assessments, develop individualized care plans, and coordinate multidisciplinary healthcare services

• Documentation Excellence: Utilize Well Sky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments
• Care Coordination: Collaborate effectively with physicians, specialists, and healthcare team members to ensure seamless patient care
• Quality Assurance: Monitor patient progress, adjust care plans as needed, and ensure adherence to clinical standards and regulatory requirements
• Family Support: Educate and support family members and caregivers in patient care techniques and safety protocols
