Key Responsibilities- Clinical Care Management: Conduct comprehensive patient assessments, develop individualized care plans, and coordinate multidisciplinary healthcare services
- Patient
Education:
Provide expert guidance on disease management, medication compliance, and health maintenance strategies - Documentation Excellence: Utilize Well Sky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments
- Care Coordination: Collaborate effectively with physicians, specialists, and healthcare team members to ensure seamless patient care
- Quality Assurance: Monitor patient progress, adjust care plans as needed, and ensure adherence to clinical standards and regulatory requirements
- Family Support: Educate and support family members and caregivers in patient care techniques and safety protocols
- Licensure &
Certifications:
Valid Illinois Registered Nurse license, Current CPR certification, Valid driver's license and auto insurance - Experience &
Skills:
