Position: PACE Nurse Practitioner — Home-Based Elder Care (Mon–Fri)

Job Description

Centra’s PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older. The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—whether that’s in their own home, with family, in long‑term care, or assisted living.

PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. Everything is coordinated under one roof with a dedicated interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, activities coordinators, and transport staff.

Centra Medical Group (CMG) is a comprehensive network of nearly 600 employed providers within Centra Health, serving 600,000 community members across Central and Southern Virginia.

Responsibilities
  • Monday – Friday 8:00 am – 5:00 pm – no weekends or holidays.
  • Dedicated team of providers, support staff, physical therapists, occupational therapists, dieticians, and social workers.
  • 5 patients per day – small volume.
  • Call occurs once every 7 weeks, 5 pm – 8 am; you are not expected to go out, calls are triaged before reaching you.
  • Opportunity for meaningful relationships with an elderly population.
Required Qualifications
  • Experience in palliative, hospice, primary care, or long‑term care is encouraged.
  • Graduate of an accredited nursing program, and Master of Science Degree in Nursing (MSN).
  • Minimum of five (5+) years of progressive experience in area of specialization.
  • Current licensure by the Virginia State Board of Nursing as a Nurse Practitioner with prescriptive authority; holds a current, active license as a registered nurse in Virginia or a current multistate RN license; maintains BLS and ACLS certifications; and holds nationally recognized certification in field of specialization.
