Home-Based LPN: Veteran Care & Wound​/Foot Visits

Summary

The VA Northeast Ohio Healthcare System is recruiting for a Licensed Practical Nurse for the Home Based Primary Care (HBPC) section within Community Outpatient Services (COPS).

Applicants pending completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

• United States Citizenship:
  Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
• Must be a graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing.
• If current VHA employee, education waived under specific laws.
• Must possess full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, Commonwealth, or District of Columbia.
• Valid Driver's License may qualify under Grandfathering Provision.
• Must have at least one year of qualifying experience at the GS-6 level or equivalent.
• Must fully meet all performance requirements for the GS-6 LPN/LVN and resume must support this.
• Experience examples: competent care for variable patient populations, judgment, accurate recordkeeping, patient education, and emergency management.
• Preferred: 2 years nursing experience in skilled home care, hospice, adult inpatient or skilled nursing facility; interpersonal and teamwork skills; independent clinical decision-making.
• The full performance level of this vacancy is GS-07.

• Functional: ability to climb stairs, frequent walking, standing, kneeling, repeated bending, squatting.
• Transferring patients and objects may be required.
• May be exposed to infected patients and require protective clothing; may encounter combative patients.
• Must handle stressful situations and adapt priorities.
• Must complete annual employee health requirements such as TB screening.

• Conduct independent home visits for Veterans with complex medical and psychosocial needs, delivering direct nursing care and education.
• Collaborate with HBPC Interdisciplinary Team and internal/external services to manage care.
• Provide patient and family health education focused on self-management and wellness.
• Support additional teams by completing home visits as needed.
• Prioritize patient visits based on acuity.
• Utilize HBPC policies and procedures to provide autonomous, high-quality care in the home setting.
• Complete Basic Foot Care Course within first year.
• Perform traditional HBPC home visits, wound care visits, and specialized podiatry LPN home visits.

• Work Schedule:
  
  Monday–Friday 8am–4:30pm.
• Paid Time Off: 37–50 days per year (including leave categories).
