Responsibilities
  • Supports physicians in Division of Cardiovascular Electrophysiology Medicine in providing patient care that is safe, effective, patient-centered, timely, efficient and equitable.
  • Organizes and makes decisions to gather medical information for the division in order to facilitate the provider’s assessment of the patient in order to develop and implement an individualized patient plan of care.
  • Ensures that the patient’s medical needs are addressed; consult with the patient’s physicians as needed, coordinating plans of treatment, and advocating for the patient when necessary.
  • Fosters intra‑facility and inter‑facility working relationships to help accomplish goals. Acts as a liaison between the division, clinic nursing staff, device clinic, device reps, non‑invasive lab, EP lab, ACC.
  • Obtains and organizes medical information from OSUP system as well as outside facilities, including obtaining records/information as needed from the PCP, cardiologist, pharmacy, and directly from patient
  • Coordinates patient‑care diagnostic testing and/or interventional procedures, which has been ordered by physicians. This may include communications with the Wexner Medical Center and with other multiple outreach facilities.
  • Serves as the patient facilitator and educator for potential and/or confirmed EP patient conditions/diagnoses, treatments, and procedures.
  • Performs and coordinates initial assessments and ongoing reassessments of the patient’s status; documents patient case information within the electronic medical record (EMR); performs chart review/audit. Evaluates patient to gather basic history, accurate medication list and other clinical information. Observe patients and reporting changes in patients’ conditions, such as adverse reactions to medication . Measure and record patients’ vital signs, such as height, weight, temperature, blood pressure, pulse and respiration.
  • Help maintain patient flow and workflow. Work collaboratively as a team member of the clinic staff to provide a high quality and efficient service to patients and clinic. Recognizes patient rights and provides for confidentiality of patient information and for dignity through own performance and in relationship with others.
  • Collaborates with other disciplines; promotes and supports research activities; and utilizes available resources to provide and improve patient care. Working within scope of license, develop and implement nursing plan of care for patients.
