Field Nurse Practitioner (Care More - San Fernando Valley / Antelope Valley, CA) Region:
Palmdale, Lancaster, North Ridge, Woodland Hills, Reseda, Panorama City

Full-Time | Monday–Friday | Field-Based

We are seeking a compassionate Adult/Geriatric Nurse Practitioner to join our growing home-based care team in the San Fernando Valley & Antelope Valley area. This is a meaningful field role focused on caring for patients where they are most comfortable — at home — during some of the most important moments in their recovery.

You will manage an assigned panel of patients receiving acute and transitional care in the home
, including individuals recently discharged from the hospital, emergency department, or skilled nursing facilities, as well as medically complex, homebound patients with lower acuity needs.

• Conduct in-home health assessments and focused physical exams
• Manage acute and post-acute conditions for patients transitioning from hospital, ER, or nursing facilities
• Develop, implement, and document individualized care plans
• Order, interpret, and evaluate diagnostic tests
• Prescribe medications and treatments within scope of practice
• Collaborate closely with physicians, care managers, and interdisciplinary team members
• Educate patients and families to support recovery, safety, and long-term health at home
• Maintain timely, accurate clinical documentation

This position serves patients throughout the San Fernando Valley & Antelope Valley including:

San Fernando Valley (SFV)

• Northridge
• Van Nuys
• Woodland Hills
• Reseda
• Panorama City

Antelope Valley (northern LA County)

• Palmdale
• Lancaster

• Schedule: Full-time, Monday–Friday
• Salary Range: $ – $ annually
• Bonus eligible based on individual and company performance

• Master’s degree in Nursing (MSN)
• Adult/Geriatric Nurse Practitioner certification
• Minimum 1 year of Nurse Practitioner clinical experience (home-based or transitional care experience a plus)
• Active, unrestricted RN and NP license in the applicable state
• Current TB test or willingness to obtain prior to start
