Position Summary
Make an impact by joining Providence at Home with Compassus, leaders in home-based hospice and palliative care. We’re seeking a compassionate and experienced Nurse Practitioner to provide essential support across various care settings, including homes, long‑term care facilities, and hospitals. The role includes independent coordination, assessment, care planning, referrals, and documentation in collaboration with the interdisciplinary team. The NP works under direct physician order or approved protocols and may independently order opioids and other medications within scope of practice.
The NP will also provide education to patients, families, and staff, maintain schedules, participate in quality improvement, and cover on-call rotations.
- Complies with organization-wide policies and patient care requirements of all regulatory agencies.
- Creates a safe patient care environment and reports/ corrects unsafe conditions.
- Conducts thorough medical, functional, and psychosocial assessment on assigned patients under physician supervision.
- Performs diagnostic tests, interprets results, and determines need for immediate physician attention.
- Provides appropriate medical interventions, initiates and alters drug therapies, and writes orders per approved protocols.
- Performs Face‑to‑Face, End‑of‑Life, and recertification assessment visits.
- Collaborates with health team members to provide continuity of care, sharing patient information with physicians.
- Orders drugs and supplies, records as defined by protocol, and documents notes in the clinical medical record.
- Participates in interdisciplinary team meetings, recommends treatment plans, and monitors ongoing care.
- Provides liaison and information to physicians, discharge planning, and other departments regarding quality of care issues.
- Recommends appropriate actions based on patient condition.
- Independently orders IV therapy, including antibiotics and opioids, when needed.
- Furnishes medications per standardized procedures, including those under Providence St., Josephs Health formulary, or specialty physician prescriptions.
- Orders feeding and placement/replacement of nasogastric or gastric tubes, adjusting rate and formula per patient needs.
- Assists families and patients with decisions regarding goals, values, interventions, and resuscitation status.
- Educates patients and families about diagnosis, prognosis, and care options.
- Facilitates referral of patients to appropriate health facilities and community resources.
- Provides education to nursing staff and facility departments on special needs and elderly care.
- Establishes courteous cooperative relations with public, patients, and personnel.
- Develops and implements evidence‑based education programs.
- Demonstrates working knowledge of the Hospital Information System and performs documentation that is timely, accurate, and concise.
- Utilizes computer‑based communications and database systems.
- Participates in provider on‑call rotation and performs other duties as assigned.
- Required:
Master's Degree in Nursing. - Required:
Minimum 2 years in acute or transitional care settings.
- Required:
Active, unencumbered RN license in the state of practice. - Required upon hire:
National Provider BLS – American Heart Association (Vendor‑Managed).
The role requires regular standing, walking, manual dexterity, and lifting/moving objects up to 50 lbs. Visual acuity, including close and distance vision, color and peripheral vision, depth perception, and focus adjustment are required. The environment is a healthcare setting with exposure to bodily fluids, infectious disease, and typical clinical conditions. Routine use of standard medical equipment and tools is essential.
CompensationBase pay range: $65.87–77.88 per hour. Additional compensation may be available through shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonuses.
