What We Offer:

• Competitive salary $110,000–$137,000, based on experience.
• Full‑time permanent position with daytime, flexible schedule and on‑call rotation.

About the Position:

Our Advanced Practice Providers deliver primary, urgent, and acute care to frail elderly patients in their homes. The Nurse Practitioner will maintain accurate records, schedule appointments, and collaborate with physicians, social workers, and paramedics to deliver concierge care.

• Partner with clinicians to manage health and wellbeing of home‑dwelling patients.
• Engage patients and families in the Well Be program.
• Provide primary care, urgent care, and specialist referrals.
• Order, interpret, and analyze diagnostic tests; determine follow‑up and treatment plans.
• Prescribe and manage medications, including psychopharmacological agents.
• Maintain accuracy in documentation, coding, HEDIS metrics, and medical expense reporting.
• Deliver palliative and end‑of‑life care, and facilitate family meetings.
• Support interdisciplinary team through regular meetings and other tasks as needed.
• Maintain required licensure, certification, and driving credentials.

Must Haves

• Master’s Degree in Nursing.
• Valid nurse practitioner license in state of operations and board certification.
• Minimum 5 years of progressive care experience.
• BC/ACLS certified, NPI#, and valid driver’s license with car insurance for patient travel.

Skills & Abilities

• Strong teamwork, ethics, and integrity.
• Expert in diagnosing and treating chronic, urgent, and psychological conditions in older adults.
• Experience in geriatrics, internal medicine, palliative, and end‑of‑life care.
• Excellent verbal, written, and presentation communication skills.
• Proficient in Word, Excel, and PowerPoint.
• Home‑care experience preferred.
• Ability to manage multiple tasks and time effectively.

Travel may be required up to 100% locally.

Work conditions include lifting up to 20 lbs routinely, up to 50 lbs during patient movement, standing for extended periods, and driving to patient locations. Fine motor skills and visual acuity are required.
