Registered Nurse, Primary Care Practice, Jennersville, PA

Christiana Care is currently seeking a full-time Registered Nurse for our Primary Care Practice located in Jennersville, PA. You will be one of a team of 8 Registered Nurses supporting a team of 9 physicians and APCs. As part of a Regional Strategy, the RN will be working a rotating schedule, traveling to our Primary Care practice at West Grove, PA, and some home remote work after orientation.

Other practices within this region include Broomall, Glen Mills, Kennett Square, PA, and Greenville Primary Care Practice in DE. The regional strategy will be further explained during the interview process.

Work Schedule:

Monday-Friday, day shift.

• Demonstrate knowledge & skills necessary to provide care appropriate to pediatric, adolescent, adult and geriatric patients virtually or on-site within the practice, including knowledge of growth development, the ability to obtain and interpret information to identify patient needs and provide care needed.
• Participate in a relationship-based care approach to care delivery.
• Perform Nurse Visits such as Annual Wellness Visits, Advance Care Planning and Transitional Care Management calls and disease management.
• Perform point of care tests, EKGs and other tests as directed by clinicians.
• Triage walk-in patients and ensure appropriate disposition.
• Answer and respond to telephone messages under the direction of the provider.
• Assist providers in examinations or procedures.
• Function as a liaison between patients, physicians and staff to provide accurate communication.
• Collaborate with team members to provide smooth, organized and efficient flow of patients.
• Respond to patient crisis situations.
• Assure adequate coverage within the unit when needed.
• Provide patient education and wellness counseling.
• Perform medication history.
• Support the work of LPN and/or MA.
• Assist with staff education and evaluation of clinical competencies for new and existing employees (MA & LPN).
• Assist with orientation of new personnel to ensure they are properly trained to support patient and provider.
• Perform chart audits or have designee assist with chart audits.
• Randomly shadow clinical support staff to ensure appropriate care is provided.
• Support the site by acting as the liaison for JCAHO, POCT, Tracer monitor for Safe Practice Behavior Monitoring (SPBM) tools & JCAHO Tracers.
• Administration of medications, monitoring of vital signs, and performing routine treatments and procedures prescribed by the physician when needed.
• Keep current with policies and procedures and participate in quality improvement activities.
• Participate in special projects.

• A graduate of a state approved or National League of Nursing accredited School of Nursing.
• RN compact license required.
• Two years’ experience as a RN, physician office experience preferred.
• BSN preferred; ADN considered with agreement of completion of BSN in 3 years.

Hourly Pay Range: $38.22 - $61.16
