* + Graduate from an accredited Nurse Practitioner or Physician Assistant program

+ Valid and unrestricted California Nurse Practitioner or Physician Assistant license

+ Active DEA registration number

+ BLS and ACLS certification

+ 1+ years of recent relevant experience preferred
