Job Description - RN Patient Navigator, Valve and Structural Heart (2600046Q)
What You Will DoThe Valve and Structural Heart RN Navigator coordinates and manages patient referrals, including contacting patients, completing intake documentation, scheduling appointments, and ordering procedures, laboratory work, and other diagnostic testing. This role is responsible for following up with patients to ensure all required testing is completed in a timely manner.
They also manage Structural Heart meeting documentation to ensure patients are reviewed by the multidisciplinary team, communicate team recommendations to patients, coordinate procedures, and support the ongoing plan of care.
- Participate and collaborate within assigned program(s); staff, leadership and committee at hospital entity and with UH system initiatives.
- Monitor and guide patients through the complete experience from pre‑op through post‑op, performing clinical screening, coordinating evaluation, scheduling appointments and testing/imaging, preparing for procedures, and facilitating rehabilitation as needed.
- Facilitate interdisciplinary collaboration in each patient care setting (hospital, home care, outpatient) to achieve optimal outcomes and satisfaction.
- Monitor and troubleshoot key patient touch‑points along the continuum.
- Document delegated portions of care/interventions in the ambulatory and/or inpatient medical record.
- Participate and collaborate with Patient Education to develop and monitor informational and educational material/opportunities.
- Support and serve as a resource to patients/families.
- Support operational initiatives.
This hybrid role requires approximately two days per week onsite at CMC, with the remaining days following the physician to clinic or working from home. Hours are flexible Monday through Friday, with occasional late clinic runs.

- Abides by all requirements to safely and securely maintain Protected Health Information (PHI); annual training and UH policies apply.
- Bachelor of Science in Nursing (BSN) – Required
- Associate Degree in Nursing (ADN) – Required
- 5+ years in clinical medicine – Required
- 3+ years in Cardiology – Required
- Ambulatory nursing experience – Preferred
- Current knowledge of cardiovascular disease management, trends, care across practice settings – Preferred
- Understanding of hospital discharge process (DME, home care, skilled nursing) – Required
- Self‑directed and motivated – Required
- Detail oriented – Required
- Strong communication skills – Required
- Familiarity with Microsoft Office – Required
- Registered Nurse (RN), Ohio and/or Multi‑State Compact License – Required upon hire
- BLS – Required
- ACLS – May be required based on clinical area standards
- PALS – May be required based on clinical area standards
