We are seeking a compassionate and experienced Hospice Registered Nurse Case Manager to provide high-quality hospice care to patients in their homes and senior living settings. This is a full-time, field-based nursing role where the RN will manage an assigned hospice patient caseload, coordinate care plans, support patients and families, and work closely with the interdisciplinary care team. The ideal candidate has prior hospice experience, strong clinical judgment, and is comfortable traveling throughout the day to visit patients in their homes.

This position is with a well-established home health and hospice organization that has served patients for many years and offers long-term stability, strong benefits, and a supportive care environment.

  • Manage an assigned hospice patient caseload and coordinate individualized plans of care
  • Perform patient assessments, monitor changes in condition, and provide appropriate interventions

  • Collaborate with physicians, aides, social workers, chaplains, and other members of the interdisciplinary team
  • Complete timely and accurate documentation using electronic charting

  • Travel throughout the service area to complete daily patient visits
Requirements
  • Active Registered Nurse license in the state
  • Prior hospice experience required
  • Comfortable providing care in home-based and senior living settings
  • Ability to manage a hospice caseload independently
  • Strong communication, documentation, and patient education skills
  • Must be willing and able to travel throughout the day to complete patient visits
  • Must be a U.S. citizen or green card holder

  • Short-term disability
  • Life insurance
  • Paid mileage

Schedule and Travel Expectations

This is a full-time position. This is not a traditional shift-based role. The RN will visit multiple patients per day in their homes or senior living facilities. Travel is a regular part of the role and is included in the salary, with additional mileage reimbursement provided.
