Role Overview

The Registered Nurse, Virtual Care provides high-quality clinical support to patients and caregivers through virtual care platforms. Reporting to the Nurse Cohort Manager, this role is responsible for delivering consistent, patient-centered care, maintaining excellent clinical documentation, and collaborating across care teams to ensure continuity and quality outcomes.

This is a fully remote role designed for nurses who thrive in a fast-paced, mission-driven virtual care environment.

Full-Time- 36–40 (Option for 4, 6, 8, and 10-hour shifts)

Key Responsibilities

Clinical Care & Patient Support

  • Deliver virtual nursing care using evidence-based clinical judgment and the nursing process
  • Conduct comprehensive health assessments, triage patient concerns, and provide clinical guidance across multiple care settings
  • Promote patient health, safety, and wellness through proactive outreach and education
  • Provide education and clinical support to family and professional caregivers

Communication & Care Coordination

  • Build strong rapport with patients, caregivers, care coordinators, and partner organizations in a virtual environment
  • Communicate clearly, compassionately, and professionally with all stakeholders
  • Identify and elevate urgent or concerning clinical findings according to established protocols

    Collaborate with internal care teams to ensure timely follow-up and continuity of care

Documentation & Quality

  • Maintain accurate, timely, and thorough documentation in the electronic health record (Athena)
  • Ensure documentation meets Pocket

    RN clinical standards, quality expectations, and regulatory requirements
  • Adhere strictly to HIPAA and patient privacy standards in all virtual interactions

Professional Practice & Team Engagement

  • Follow Pocket

    RN workflows, clinical guidelines, and escalation pathways
  • Actively participate in coaching, performance feedback, and ongoing education
  • Contribute to continuous improvement initiatives and support a collaborative team culture
  • Demonstrate accountability for schedule adherence, attendance, and professional conduct
Qualifications

Education

  • Graduate of an accredited school of nursing
  • Current BLS certification

Experience

  • Minimum of 3 years of direct patient care experience
  • Experience in geriatrics, primary care and dementia care strongly preferred
  • Experience in telephone triage is preferred

Licensure

  • Current, active, and unencumbered RN license required
  • Licensure in all 50 states required

Skills & Competencies

  • Strong clinical judgment, critical thinking, and assessment skills
  • Excellent verbal, written, and asynchronous communication skills
  • Ability to deliver empathetic, patient-centered care in a virtual setting
  • Comfortable working independently while remaining engaged with a remote team
  • Adaptable and eager to learn in a fast-paced health-tech startup environment
Technical &

Work Environment Requirements
  • Proficiency with EHR systems (Athena preferred), Apple/macOS proficiency and Google Workspace tools
  • Reliable high-speed internet connection (minimum 50 Mbps). For testing your internet speed, visit
  • Quiet, private home workspace suitable for HIPAA-sensitive video calls
  • Laptop or desktop computer (Chromebooks not supported)
  • Additional external monitor required
  • Evening and weekend shifts may be required
