Remote House Calls Nurse Practitioner – Job Description
Intention Healthcare, an Essen Health Care company, delivers an integrated model of in‑home primary care, longitudinal care management, remote monitoring, and real‑time support for caregivers. We are looking for a Remote House Calls Nurse Practitioner (NP) in NJ, OH, PA, NE, TX to provide virtual clinical oversight, patient assessments, care coordination, and provider support for patients receiving care in their homes.
Responsibilities- Conduct telehealth visits for patients enrolled in the House Calls program.
- Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes when primary care services are required.
- Perform care‑management needs assessments, follow‑ups, and adjust care plans.
- Review patient histories, labs, imaging, and diagnostic results.
- Support medication management and reconciliation.
- Coordinate care with primary care providers, specialists, and care‑management teams.
- Document visits and clinical decisions accurately in the EMR.
- Ensure compliance with clinical protocols, quality standards, and regulatory requirements.
- Participate in interdisciplinary case reviews and quality improvement initiatives.
- Active Nurse Practitioner (NP) license in the required state(s).
- National NP certification (ANCC or AANP).
- Minimum 2+ years of clinical experience as a Nurse Practitioner.
- Experience in house calls, home health, geriatrics, primary care, or internal medicine strongly preferred.
- Prior telehealth or remote care experience preferred.
- Strong clinical judgment and comfort managing complex patients remotely.
- Excellent communication and documentation skills.
- Proficiency with EMR/EHR systems.
- Comfortable using telehealth platforms and remote monitoring tools.
- Active DEA registration (or willingness to obtain, if applicable).
