Salary:$1,200 USD/month (depending on experience)
We are seeking a licensed nurse to support a U.S.
-based endocrinology practice through its Principal Care Management (PCM) program
.
PCM is a care coordination program designed for patients with a single chronic condition requiring ongoing management
. In this role, you will work closely with the provider to ensure patients receive continuous support between visits, helping improve outcomes and adherence to treatment plans.
This is a clinical, patient-facing role
, requiring strong judgment, communication skills, and the ability to actively monitor and guide patients throughout their care journey.
- Maintain ongoing communication with patients enrolled in the PCM program
- Monitor patient conditions, including:
- Symptoms
- Medication adherence
- Progress and response to treatment
- Provide patient education regarding their condition and care plan
- Assist in the implementation and follow-up of care plans
- Coordinate services and ensure continuity of care
- Identify changes in patient status and escalate concerns to the provider when needed
- Ensure patients follow treatment recommendations and scheduled appointments
- Accurately document all patient interactions and updates in the system
- Advanced English level (C1 or higher) – excellent spoken and written communication
- Proven experience working with U.S.
-based medical offices - Experience with chronic care management, patient follow-up, or care coordination
- Strong clinical judgment and ability to assess patient conditions remotely
- Experience providing patient education and support
- Highly organized with strong documentation skills
- Comfortable working independently in a remote environment
- Reliable computer (8GB RAM minimum)
- Quiet and professional workspace
