Schedule:

Full-time (40 hrs./week). On-call may be required based on branch needs. Overview We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their place of residence and coordinate care with an interdisciplinary team.

Key Responsibilities Perform comprehensive home health assessments (stable and emergent situations) Develop, implement, and update the Plan of Care (POC) Provide accurate, timely documentation and maintain up-to-date patient records Educate patients/families on medications, safety, disease management, and self-care Coordinate care with physicians, care team members, and referring partners Supervise/direct support staff as applicable (HHA/STNA/LVN) Provide on-call coverage when assigned Use infection control practices and maintain compliance with payer requirements Required

Skills & Qualifications Active RN license (WA) (BSN preferred) 1-2 yrs’ clinical care & 2+ years Home Health RN experience (required) Homecare Homebase (HCHB) & OASIS experience-required Current CPR certification Valid driver’s license + reliable transportation + insurance Comfortable with wound care, IV/venipuncture, catheter/drain care, & DME use Preferred
