Devoted Health, Inc. is looking for a Bilingual (English/Spanish) Transitions of Care RN Case Manager to provide telephonic, short-term, interdisciplinary care management for high-risk patients post-discharge. This fully remote position requires an unrestricted RN license and 3-4 years of relevant experience. Responsibilities include patient engagement, care plan development, and coordination with other healthcare providers. The salary range is $85,000 to $100,000 annually with various employee benefits including health insurance and paid time off.
